Description: Physician names, business and practice addresses, national license numbers, phone and fax, and physician taxonomy (their specialty). Original dataset (link) is an extremely large file; my dataset narrows it down to Illinois only.
Prior use of data: We have used this data at CHITREC to look at provider density in Chicago.
